There are two steps involved in receiving (Regular) Employment Insurance Benefits, while on lay-off.
Step 1– Is to apply for (Regular) Employment Insurance with an online application and use our reference code (found on our website)
Step 2- Four or Five days after you have submitted your application, you will receive a 4-digit Personal Access Code to your mailing address. This will allow you to complete your Employment Insurance Reports and receive Employment Insurance Benefits for weeks you are laid-off

Guide to questions being asked while reporting
The guide below is specifically for Step-2 -Employment Insurance Reporting.
However this is just a guide. All answers are subject to your personal situation and should be answered accordingly.
ACCEPTANCE STATEMENT
Read over the statement and choose “I ACCEPT”
ADDRESS AND DIRECT DEPOSIT
Question #1: Have you moved, changed mailing address, or changed your banking information?
Answer #1 NO or YES as it applies to you
ADDRESS AND DIRECT DEPOSIT CONFIRMATION
If the answers you provided are correct – choose “I ACCEPT”
OUTSIDE CANADA
Question #2: Were you outside of Canada during the period of this 2 week report?
Answer #2 NO or YES as it applies to you
Note: Almost everyone should answer NO, unless they were out of the Country during the lay-off period in question
OUTSIDE CANADA CONFIRMATION
If the answers you provided are correct – choose “I ACCEPT”
WORK and WAGES
Question #3: Are you self-employed?
Answer #3: NO or YES as it applies to you
Note: For the majority that are only employed at the Ford Motor Company or who may also have part-time employment at another employer? You would indicate NO
However, if you are self employed you would indicate YES, and be asked to answer a list of other questions not addressed here.
Question #4 : Did you work or receive earnings during the period of this report?
Answer #4: NO or YES as it applies to you
IMPORTANT NOTE– Make sure you carefully review the dates during the period of your report. In most cases you will be reporting for a two-week period.
Therefore if during one or both of the weeks you worked, received vacation, holiday or Short Work Week pay or earnings from another employer you would answer YES.
If during the the two week period you had no earnings (other than a Supplemental Unemployment Insurance Benefits, which are not considered earnings for Employment Insurance purposes). You would answer NO.
Question #5: Did you start a full time job during the period of this report?
Answer #5 : Everyone should answer NO
Note: This is because we are not yet back to work. Once you have returned to work, you can answer YES and provide the date that you returned to regular full time employment.
If you indicated YES to Question #4? Meaning you did work or receive earnings during the period of this report, you will be asked the following questions.

Answer: NO or YES as it applies to you
If NO, you will move to WEEK 2
If YES, you will be asked
Question: How many hours did you work?
Answer: – Enter your hours you were paid or worked
Question: What was the gross amount of pay received?– Enter your gross pay (rate of pay * hours worked or paid)

If NO, you will move to EMPLOYER INFORMATION
If YES, you will be asked
Question: How many hours did you work?
Answer: – Enter your hours paid or worked
Question: What was the gross amount of pay received?– Enter your gross pay (rate of pay * hours worked or paid)
EMPLOYER INFORMATION
Question: How many employers did you work for?
Answer: Choose the number that applies to you (for most it will be 1). Then provide the phone number for each or your employers. Note: For the majority it will only be Ford Motor Company and the number is (905) 845-2511
STOPPED WORKING
IMPORTANT NOTE: This is one of the trickiest questions for people, read carefully. The question is asking if there is any other employment you have lost that you have not already told EI about.
Question: Have you stopped working for any employer during this report?
Answer: should be NO
Note: The answer for most should be NO, as the Ford Motor Company is the only employer they have been laid-off from during this period.

TRAINING
Question #6: Did you attend school or a training course?
Answer # 6: NO or YES as it applies to you
Note: For most it will be NO, unless in the unlikely event you are attending school or a training course that interfered with your ability to work full time.
TRAINING CONFIRMATION
If the answers you provided are correct – choose “I ACCEPT”
AVAILABILITY
Question #7: Were you ready willing and able to work during the period of this report?
Answer #7: NO or YES as it applies to you
Note: For most the answer will YES, unless you are unable to work due to disability/illness or are not willing to work if any other reason.
AVAILABILITY CONFIRMATION
If the answers you provided are correct – choose “I ACCEPT”
OTHER MONEY
Question #8: Our records show you are part of a SUB program, is there any *other money that you have not told us about?
IMPORTANT NOTE: This is another tricky question that some people have trouble with. Whether or not you are entitled to a SUB payment, you are still part of a SUB plan. Service Canada is aware of this and as indicated earlier you should never declare Supplemental Unemployment Benefits as earnings or money. You only need to report other income if you receive any.

Therefore the answer for almost everyone would be NO

Print if you wish to retain a paper copy of your report
Then “SIGN OUT”
Your report is complete. Service Canada will now provide you with information as to when you can complete your next report.
IMPORTANT NOTE: Complete the required Employment Insurance report within 3 weeks of that date to ensure you remain eligible for Employment Insurance Benefit payments and your report is not considered late.
