Optional Life Insurance/ Dependent Life Insurance Open Enrollment

The recent (2020) Agreement between the Company and the Union provides for an open enrollment
period effective May 1 to May 31, 2023 which allows employees the opportunity to increase
their coverage in the employee-paid Optional and/or Dependent Life Insurance Program
without evidence of insurability.


Employees currently enrolled in the Optional/Dependent Life Insurance Program

Any employee actively at work and currently enrolled in the Optional/Dependent Life Insurance Program may increase the amount of insurance up to one (1) Schedule, without
evidence of insurability.


Employees not currently enrolled in the Optional/Dependent Life Insurance Program


Any employee actively at work and who had not previously enrolled in the Optional/Dependent Life Insurance Program has the opportunity to enroll at Schedule 1, without evidence of insurability.


For the open enrollment listed above, the coverage will become effective by July 1, 2023 provided that the employee is actively at work on such date.
If you wish to take advantage of this Open Enrollment, please see your Company Benefit Representative to obtain the enrollment form. Enrollment forms must be completed, signed and returned to your Company Benefit Representative no later than May 31, 2023.


If you wish to select insurance coverage not covered under the open enrollment (i.e. increase coverage more than one Option Level) please see your Company Benefit Representative to obtain the “Statement of Physical Condition” form. Return this form together with the enrollment form to your Company Benefit Representative. These applications will be subject to acceptance by the insurance carrier. Coverage will become effective the first of the month following approval by the insurance carrier and payment of the first premium.
